Please take a few minutes to fill out your paperwork here. This will allow us to process all the information and be ready for your first visit. On your first visit to our office, we will have your completed form available for your signature. The security and privacy of your personal data are one of our primary concerns and we have taken every precaution to protect it.
If you do not wish to complete your paperwork online (here), you should arrive 10-15 minutes early so you can fill out the paper version.

I always have a great experience every time I come to this dentist. Their facilities are clean and modern and I love the TV's you can watch above you while having your teeth worked on (it's a nice distraction!). Both Caroline and Dr. Patel are super friendly and gentle when doing their work. It's also nice to receive text message reminders about your upcoming appointment and I like the free water offered at the front.
